Do I need to apply for anything before I travel to Lithuania?

No — with a Paraguay passport, you don't need to apply for a visa or authorisation before flying to Lithuania. Just make sure your passport itself is valid and, if you have one, bring proof of your onward or return travel in case you're asked.

Can I extend my stay in Lithuania once I'm there?

Only in limited circumstances, and not simply to keep travelling. You must apply before your current permission expires, and where a fee applies it is €30. Only up to the 90-days-in-180 ceiling. Going beyond it requires force majeure, and the resulting sticker is usually valid only in the country that issued it. Free where the grounds are force majeure or humanitarian. The €30 applies to serious personal reasons, or to an entry delayed through no fault of your own. Apply to the immigration authority of the Schengen state you are currently in — not the consulate that issued the visa — before your visa expires. An expired visa cannot be extended. If you know in advance you need longer than 90 days, the route is a national long-stay (Type D) visa, applied for from outside the Schengen Area before you travel.
